Dear Parents/Guardians of all LAKE ELEMENTARY SCHOOL students:
Please complete the following form as soon as possible for EACH STUDENT in your household/family. This information will be used to ensure that we have accurate, up-to-date contact information on every student until registration forms are completed and returned to our school site. Each student will receive a registration packet on the first day they attend school. The packet will need to be returned with your child(ren) on their second day of attending school.
